  • Our consultation hours are by appointment. An appointment is for 1 complaint and takes a maximum of 15 minutes. When making an appointment, the assistant will ask you what you are coming for. The assistant is trained to estimate how quickly you need care based on your complaint. She can therefore make an appropriate appointment for you. If you do not wish to discuss this with her, please estimate how much time you will need so that wait times can be reduced.

  • The home visit is intended for people who cannot physically come to the regular consultation hour. For example, because you are too sick, (temporarily) disabled or have difficulty walking. You must request a home visit in advance from the assistant. She will make an appointment for you in the agenda of General Practitioner. There are fixed times for requesting home visits. Call before 10 am. Please keep this in mind.

    If at all possible to come to the practice, you should still do so. Treatment options at the practice are much greater than at your home. Often the General Practitioner will be able to help you better and faster in the practice.

    In specific cases, the physician assistant also makes home visits.

  • Assistant consulting hours

    You can contact the assistants for many practical matters such as:

    • advice regarding simple medical conditions

    • referral cards

    Diabetes consultation

    Diabetes patients are monitored and supervised by the somatic practice support worker. The primary care assistant has her own consultation hours (under the responsibility of the general practitioners) and sees the patients several times a year.

    Asthma-COPDS consultation

    The Somatic practice assistant also has consultation hours for asthma and COPD, for which patients are called once or twice a year.

    Cardiovascular disease

    Patients with an increased risk of cardiovascular disease are seen by our Somatic Practice Assistant. Think of people with high blood pressure or high cholesterol. In the context of Cadiovascular Risk Management, these patients are checked, a physical examination is done and - if necessary - blood tests. Then, in consultation with the patient and the General Practitioner, we look at what things can be changed to positively influence the health of the patient.

    Eldercare

    Our somatic nurse practitioner contacts people aged 75 and older to identify early problems. Think of problems in the areas of loneliness, mobility, housing or memory. By looking at these issues early, help can sometimes be provided. Before bigger problems arise.

    Surgical procedures

    Minor surgical procedures can be done at our practice. Examples include the removal of small moles or fat bumps. The General Practitioner does want to have seen the patient the first time at the consultation to assess the procedure and explain the treatment.

    Reimbursement of laboratory tests

    When the General Practitioner refers you to a medical center or laboratory for examination, you will receive reimbursement from the basic insurance for the costs. These costs will be charged to your compulsory (and possibly voluntary) excess. Investigations carried out in our own practice (for example, blood sugar determination or research into anemia) are not covered by the deductible. This with the exception of examinations that we perform on behalf of the laboratory, such as a CRP test (a blood test in case of suspicion of inflammation) or taking a culture (such as urine culture or STD testing). A culture is often submitted to the practice, or taken at the practice, but then sent to the laboratory for testing.

    The General Practitioner bases the decision to request additional laboratory testing on medical arguments and can only take limited account of its cost, which is within your deductible.

  • If you think you have a bladder infection, you can submit urine to the assistant. Please call the practice for this first so we can ask about your symptoms.

    Preferably morning urine. If you hand it in before 9:30 a.m., you can call after 2 p.m. for the results. Please clearly mention your name and date of birth on the jar. You can get jars at the practice.

  • The costs will be reimbursed without drawing on your deductible fully reimbursed by the health insurance company when:

    1) you visit the General Practitioner /doctor's assistant/practitioner GGZ.

    2) it makes a visit to you.

    3) a telephone consultation takes place.

    When you visit the POH somatics for, for example, guidance of your diabetes, cholesterol and/or blood pressure, asthma/copd, this does at the expense of your deductible and any voluntary deductible excessas you have agreed with your health insurer.

    For a referral to a specialist or for additional tests, such as blood tests, X-rays, (urine) / (stool) / (STD) cultures, or pathology tests you also pay your (voluntary) deductible excess.

    Medication you are prescribed by the General Practitioner will also at the expense of your (voluntary) deductible.

    These are the general rules, which your General Practitioner is not always fully aware of either. Sometimes there are changes or exceptions. Your health insurance company can tell you more about them.

  • General

    The AVG is the new law to protect privacy and personal data. Under this law, an organization that works with personal data has certain duties and the person whose data belongs has certain rights. In addition to this general law, specific rules apply to privacy in health care. These rules are stated, among other things, in the Medical Treatment Agreement Act (WGBO). These privacy regulations are intended to inform you of your rights and our obligations under the AVG and the WGBO.

    General practice

    Our general practice may process various personal data about you. This is necessary to be able to treat you properly medically and necessary for the financial settlement of the treatment. In addition, processing may be necessary, for example, to combat serious danger to your health or to comply with a legal obligation (for example, the mandatory reporting of an infectious disease under the Public Health Act).

    The duties of general practice

    Huisartsenpraktijk Feanstate is, according to the AVG, the controller of the processing of personal data that takes place in the practice. The practice complies with the resulting duties as follows:

    Your information is collected for specific purposes:

    for care delivery;

    for effective management and policy;

    to support scientific research, education and outreach.

    In principle, no processing takes place for other purposes.

    You will be informed that personal data about you is being processed. This may be done by your health care provider, as well as through a leaflet or through our website.

    All employees within Feanstate are committed to handling your personal data confidentially.

    Your personal data is properly secured against unauthorized access.

    Your personal data will not be kept longer than necessary for proper care.

    For medical data this retention period is in principle 15 years (from the last treatment), unless longer retention is necessary, for example for the health of yourself or your children. This is at the discretion of the practitioner.

    Your rights as a data subject

    You have the following rights:

    The right to know whether and what personal data about you is being processed.

    The right to inspect and copy that data (to the extent that it does not violate the privacy of another).

    The right to correct, supplement or delete data if necessary.

    The right to request (partial) destruction of your medical data. This can only be complied with if keeping the data is not of significant interest to another person and the data does not have to be kept under a legal regulation.

    The right to add a self-declaration (of a medical nature) to your file.

    The right to oppose the processing of your data in certain cases.

    If you want to exercise your rights, you can make this known to your General Practitioner orally or using an application form. Your interests may also be represented by a representative (such as a written agent, or your guardian or mentor).

    Explanation of application form

    You should keep in mind that medical records are basically kept for a maximum of 15 years under the law. You help us to look up your records and protect your privacy if you fill out the form as completely as possible. The data you enter will be treated strictly confidentially by us. Huisartsenpraktijk Feanstate is not liable for errors in mail delivery. If you prefer to pick up the file in person, or by an authorized representative, please indicate this on the form.

    Patient data

    Please include the details of the person about whom the medical record is concerned. The Medical Contract Act (WBGO) considers the patient to be of age from 16 years of age. Adolescents from 16 years of age who wish to inspect/remember their medical records must make the request themselves. If the patient is no longer alive, provision of the medical records is permitted if it can be assumed that the deceased would not have objected or there are compelling interests to break the healthcare provider's duty of silence. This decision lies with the healthcare provider.

    Provision of your personal data to third parties

    The employees of Feanstate have an obligation to treat your personal data confidentially. This implies, for example, that the health care provider needs your explicit consent before providing your personal data. However, there are some exceptions to this rule. On the basis of a legal requirement, the care provider's duty of confidentiality may be broken, but also when there is reason to fear a serious danger to your health or that of a third party. Moreover, if necessary, recorded data can be exchanged orally, in writing or digitally with other healthcare providers (e.g. the pharmacist who processes a prescription and thus receives data from the attending physician).

    Data exchange

    After you have given your specific permission, Feanstate GP practice exchanges relevant medical data safely and reliably with the primary care physician's clinic (HAP) via the National SwitchPoint. If you have visited the HAP in the evening or at the weekend, the HAP in turn shares a truth message with the GP practice. This way the General Practitioner knows exactly what complaints you visited the HAP with and what action was taken in response.

    Medication data may also be shared with your pharmacy and your treating medical specialists. This includes the medication prescribed to you by the General Practitioner , as well as any intolerances, contraindications and allergies (ICA data). These can be taken into account by other prescribers and providers of medication. This is how we as a general practice contribute to medication safety.

    Transfer of your file

    If you choose a new doctor/health care provider, it is important that your new doctor/health care provider is aware of your medical history. Your medical history is in your patient record. It is customary for your old doctor/health care provider to transfer the record to your new doctor/health care provider. The old doctor/health care provider does this as soon as possible , at least within one month, after you have asked your old doctor/health care provider to transfer the record to your new doctor/health care provider.

    Your medical record will then be transferred by your doctor/health care provider either in person or by registered mail. You cannot take the original file with you. However, you do always have the right to see your file and to receive a copy of your file.

    The file (if possible) can also be transferred to the new doctor/caregiver via e-mail. Both doctors/caregivers must then ensure that their computers and Internet connections are sufficiently secure.

    Question or complaint

The Complaints Officer will work with you to find a solution. The Complaints Officer can also mediate in the event of a complaint.     If you are unable to come to an agreement with both your General Practitioner and the complaints officer, you can request a ruling on your complaint from the GP Care Disputes Committee. This independent committee consists of a chairman (a lawyer) and members on behalf of the patients and members on behalf of the general practitioners. The committee is assisted by an official secretary who is also a lawyer.
